Coordination of Care for Youth with Social, Emotional or Behavioral Challenges

The Community Service Agencies at Beth Israel Lahey Health Behavioral Services work with families who have children facing a severe emotional, behavioral or mental health challenge. We help you access critical services and support.

Our CSAs use a “wraparound” model of care to help children and families thrive in their homes and community. Our wraparound model offers a comprehensive, holistic, youth- and family-driven approach to responding when a child or young adult experiences serious mental health or behavioral challenges.

Our team of intensive care coordinators and family partners work with youth and families to create their team, including professional and natural supports, to ensure their ideas, vision and goals are incorporated in the plan of care. The goal of wraparound services is to help youth with complex needs remain at home.

Wraparound Services

The wraparound model facilitiates intensive care coordinators and family partners working together to put the family’s plan into action, monitor how well it’s working and change it as needed.

Our services include:

  • An individual plan and set of goals for each family, including assessing the families’ strengths and needs
  • Training and coaching for the parent/caregiver on how to navigate the child-serving systems
  • Identification of available services and support in the community
  • Help with developing connections with self-help or support groups
  • Support and coaching for parents
